Description:Evaluates patients. Reviews medical records and other information. Establishes treatment plans and goals. Provides treatment, functional training, and education. Modifies or discontinues treatment appropriately. Documents results of evaluation, treatment plan, and goals. Documents care provided and patient's response. Maintains required records. Communicates with physicians, nurses, and others on patient care. Consults with other patient care providers, the patient, and family. Reads medical and professional literature. Delegates appropriate duties and supervises performance. Develops home programs. Locates resources for treatment and equipment for patients. Identifies need for replacement and acquisition of departmental equipment and supplies. Orders individual patient care supplies. Participates in patient care conferences, clinics, and rounds. Provides leadership in developing/implementing/maintaining programs to improve the quality and variety of care for out-patients. Prepares and presents educational programs for peers, other health care personnel, students, and the public. Participates in continuing education activities. Instructs, evaluates, and counsels physical therapist and physical therapist assistant students and volunteers, as assigned. Completes required paperwork related to clinical education. Assists in orienting and upgrading skills of other staff. Sets up and cleans up patient care areas and equipment. Performs duties of supervisor and subordinates as requested. Performs selected clerical duties as needed. Schedules patient care activities for self and subordinates. Participates in Performance Improvement activities. Interacts with agencies and individuals outside the hospital. Performs other duties as assigned.

Qualifications:Bachelor's degree with a diploma in physical therapy from a program accredited by the American Physical Therapy Association or equivalent.
MINIMUM REQUIRED EXPERIENCE At least two years of professional experience as a licensed physical therapist is required.
